Documentation needed
Player Photo
Looking straight at the camera, shoulders to top of head, nothing on the face or head (no eye black, hats or hoodies).
Birth Certificate
A copy is fine.
Medical & PIAA forms
Pop Warner Medical Form and PIAA Form (Section 6) — both must be signed for the current season.
Participant Contract & Parental Consent
Must be signed for the current season.
Last Report Card
Pop Warner National requires a 2.0 GPA / 70%. A Scholastic Eligibility Form is required for those who don't meet the requirement.
